New Patient Information

Blakeview Medical Centre is an AGPAL Accredited General Practice, we offer a wide range of services, referrals and a recall system for ongoing care.

We provide: Senior’s Health Assessments, Specialists, Physiotherapy, Podiatry, Asthma Plans and Diabetes checks plus much more.  Please ask your Doctor or the Reception Staff for more details on any services that you may require.

HOME VISITS

Blakeview Medical Centre provides home visits for regular patients who are unable to come into the surgery.  This must be organised with your regular doctor.  Please ensure you ring as early as possible so staff can contact the necessary doctor to make arrangements.

AFTER HOURS

If you require medical attention after hours please phone the surgery’s phone number and you will be diverted to GP Solution’s or Phone them direct on 08 83623311.

CANCELLED APPOINTMENTS

Please notify the surgery as early as possible so that the appointment time can be reallocated, or a $10-00 fee may apply and will be billed accordingly. This fee is Not refundable through Medicare.

EXTENDED WAITING

Our Staff will make every effort to ensure that the waiting time is kept to a minimum, however delays such as emergencies and more difficult consultations are some times unavoidable.

STERALISED INSTRUMENTS

All instruments which are used at this Surgery are all  individually wrapped, single use, instruments and are disposed of after use. These are purchased from Atlas Health Care.

PRESCRIPTIONS

In the interest of better health care, we will not repeat prescriptions unless we see the patient or you have organized this previously with your Doctor.
There is a 7 day pharmacy (Priceline Pharmacy) opposite the surgery at the Munno Para Shopping Centre. Hours are from 9am to 9pm

ACCOUNTS: Bulk Billing

All consultations are bulk billed, except for Medical Examinations, which are not claimable through Medicare.  For any Medical Examinations payment is required immediately.  If an account is required then a letter will need to be provided stating the Company who will be paying.

For Medicare Accounts you must have a current Medicare Card or payment will be expected at the time of your appointment.  No Accounts will be granted unless prior arrangements are made with management.

 

WORK COVER THIRD PARTY

We handle all Workcover and Third Party consultations.  All accounts will be handed to the patient after the consultation.  If the claim is rejected the patient is liable for all accounts incurred.

SPECIALISTS

If you have been referred to see a specialist by your GP, when making an appointment to see them, please ask their reception staff about their fees, as most Specialists charge a fee above the scheduled Medicare fee.

CONSULTATIONS

Are made by appointment only (average time is ten minutes).  If you think you may need extra time please ask our reception staff.

LONG CONSULTATIONS

Must be booked for Pre-Employment Medicals, Insurance and Drivers License Medicals, Pap Smears, Baby’s 6week Checks, several other medical conditions and Counseling etc., If you require a longer appointment please inform the reception staff.

TEST RESULTS

Patient’s are reminded that they must always  have an appointment with their doctor to obtain their test results . Results can not be given over the phone  due to the confidentiality and Privacy laws.  This is for your protection and of course the Surgery’s. Any results that are not within normal range we will contact the patient immediately by phone or by mail for you to arrange an appointment with your Doctor.
